We provide dental care, preventive services, and related health services. All treatments are subject to professional evaluation and may require informed consent.

3. Payment & Insurance

Payment is due at the time of service unless prior arrangements are made.
Insurance claims are submitted as a courtesy; patients are responsible for any balances not covered by insurance.
Late or missed payments may be subject to fees.

4. Appointment Policy

Missed appointments or cancellations with less than 24-hour notice may result in a cancellation fee.
We reserve the right to reschedule or refuse treatment if policies are not followed.

5. Website Use

Content on our website is for informational purposes only and does not replace professional dental advice.
Unauthorized reproduction, distribution, or use of website materials is prohibited.

6. Limitation of Liability

Hockley Dental is not responsible for damages resulting from misuse of information on our website or delays caused by third-party service providers.

7. SMS Terms and Conditions

Hockley Dental offers a text message-based service (the "Messaging Service"). You may opt in by providing your mobile number on one of our forms, in our office, or by texting us. By opting in, you agree to receive the following types of text messages from Hockley Dental: appointment reminders and scheduling, follow-up care, billing updates, and marketing.

You can cancel the Messaging Service at any time by texting STOP. After you send STOP, we will send one message confirming that you have been unsubscribed, and you will no longer receive text messages from us. To rejoin, sign up as you did the first time or reply START. If you forget what keywords are supported, text HELP and we will respond with instructions on how to use the service and how to unsubscribe.

Message and data rates may apply for any messages sent to you from us and to us from you, and Hockley Dental is not liable for the cost of such messages. Message frequency varies. Carriers are not liable for delayed or undelivered messages. For questions about your text or data plan, contact your wireless provider. You understand that anyone with access to your mobile phone may be able to view the messages you receive, and you agree that Hockley Dental will not be liable to you if this occurs. By agreeing to these terms and providing your mobile phone number when you opt in, you authorize Hockley Dental to contact you by text message at that number using an automatic telephone dialing system or other computer-assisted technology. 8. Modifications

We may update these Terms & Conditions at any time. Updates will be posted on our website with a new effective date.
